Program Overview
PulmoDynamics Care LLC provides a structured COPD telehealth follow-up program led by Denay White, RRT, CPDE, a Certified Pulmonary Disease Educator through the AARC. This program supports patients during the critical post-discharge period by offering respiratory education, inhaler coaching, airway clearance guidance, symptom monitoring, and early identification of exacerbation signs. The goal is to improve patient outcomes, increase self-management skills, and reduce 30-day COPD readmissions.
What’s Included
The COPD Telehealth Program provides scheduled virtual support and respiratory guidance for recently discharged COPD patients. Services include 48-hour post-discharge telehealth visits, weekly follow-up sessions, inhaler and nebulizer technique coaching, airway clearance instruction, oxygen and equipment troubleshooting, symptom-monitoring, pulmonary self-management education, and smoking cessation support. All sessions are conducted virtually for patient convenience and timely follow-up.
Documentation & Reporting
After each telehealth visit, detailed documentation is completed and securely returned to the referring facility. Reports include symptom assessments, inhaler technique updates, equipment checks, patient education provided, adherence notes, red-flag symptoms if present, and recommendations for provider escalation. All information is handled in compliance with HIPAA standards to ensure patient privacy and secure communication.

Documentation is sent to the referring provider after each visit, and a final summary is delivered at the end of the program.
